FIRST YEAR ENROLLMENT: We thank you for the time it takes to complete all the required forms and fees. Download, print, and fill in all the appropriate forms and send them to 2006 Lincoln Court, Oxnard, CA 93033. Pay your fees. Photocopy all of these forms for your own personal records. Use this checklist.

CONTINUING ENROLLMENT: Thank you for continuing your enrollment with El Shaddai Academy. Returning families pay one-half of the regular registration - $50. Families of returning students, 5th grade and above, must include yearly SAT TEST RESULTS administered in the prior spring. Alternate test options are available for 11th and 12th graders (check high school section of the website). Returning families are required to complete only the forms with a red asterisk *. Many of them can be updated from last year's records. For example, for a high school student, find the photocopy of your Extra-Curricular and Community Service Record and update it with any new information for this year. Use this checklist.

Family Registration Application *

Student Enrollment (each child) *

Parental Agreement: * This form must accompany your enrollment packet. Enrollment will not be complete without this agreement signed.

Medical Release: * California law requires we have this on file. This form is kept in the school’s file, not the student’s file. Once your student leaves this school, the emergency medical form is discarded. Fill it out completely.

Faculty Information: California law requires each school to have information on each of their teachers. You, the parent, are the teacher. This form is self-explanatory. Be creative with the teacher training section. Mention any teaching type positions you’ve held (VBS, Sunday School, a team teaching situation) or any teacher training you have had (home school workshops, seminars, tapes, or books read can be noted). This is a formality.

Student Record Release: If your child has attended another school, we will need to send for his/her records. This request states the student is now enrolled in El Shaddai Academy. It does not indicate the student is being home schooled. Please fill out the complete name and address of the last school your child attended. Fill in child’s name, grade level, and date of birth. Be sure to sign your name and the date. If your child has never attended another school and you hold the records, please let us know.

Copy of Birth Certificate

Report of Health Examination for School Entry (PM-171-A): Typically this exam was performed when your student was about age six.

Photocopy of yellow Immunization Record * (update)

School Entry Requirements: If you are enrolling your student in first grade (6 years old) and he/she did not attend kindergarten at another school, we will need to establish a cumulative file.

Proposed Course of Study (grade school students) * – Basic Program Only: This form is to help you get focused for the coming year. This is another form required by California law for each school to have on file. We need these filled out and sent in with all other forms. It is permissible to make changes in your curriculum during the year. If you do make changes, please notify the school.

HSLDA Membership Application Form (new members only): Include an $85 check written to HSLDA*

MENTOR PROGRAM -- The following forms will be needed for your meetings with your assigned mentor.

Proposed Course of Study * (grade school and high school students) : At this meeting, held at the mentor's home, the teacher will present the Proposed Course of Study for each student (grade school or high school) for discussion. During this time the mentor will look over the courses planned. The mentor may offer advice or counsel, however the teacher makes the final decisions. The meeting is sealed with a prayer of wisdom for the teacher.

Course Description and Standards * (high school students) : At the mentor's home, the parent/teacher has brought her Course Description and Standards for each student (high school only) to discuss at this meeting. During this time the mentor looks over the Course Description and Standards planned by the teacher. The mentor may offer advice or bits of counsel (to tighten up or loosen up). The meeting is sealed with a prayer of wisdom for the teacher.

Registration and Enrollment Program Fees

In order to provide a service that home school families can afford, ESA has kept the enrollment fees as low as possible. Therefore, we:

  • Do not allow monthly installments.
  • Do not pro-rate registration and enrollment fees for families that enroll late in the year or refund fees to families that withdraw their students early in the year.
  • Allow families to pay be check (preferred) or by Paypal

Grade School Basic Program ($75):

  • Homeschool Reporting Online for Progress Reports on your password protected family page
  • Transference of student's records to and from another school. All records are confidential and are kept in fireproof files, as required by law, at the school office.
  • Resource site list for testing is provided upon request. Mandatory Stanford Achievement testing is required for 4th grade and up.
  • Student and Teacher picture ID cards. Call VIP to schedule your appointment (805-385-9720).

Grade School Mentor Program ($115):

  • All of the above Basic Program
  • Private Proposed Course of Study meeting with mentor.
  • Four group Progress Report meetings with Mentor
  • Paper Recording option of $20 per family (fee collected by mentor) or required if the parent/teacher is unable to post grades independently at the Homeschool Reporting Online website.

* All grade school students in your family must be enrolled in the same program.

**Compliance Deposit Account (CDA) is for on time recording, on-time submission of subject samples, and attendance for MANDATORY high school mentoring meetings. If your family is up to date and consistent, you’ll have all these funds returned at the end of the school year (completion of 175 days). If your family is having difficulty with the following criteria:

  • Recording attendance $10
  • Recording of grades $10
  • Recording of remarks $10
  • Subject Samples $10
  • Attendance at MANDATORY High School Mentor meetings (not mandatory for parents of grade school students) $50

these above fees will be deducted from your CDA whenever there is delayed compliance for one or more missing criteria. Being delinquent beyond 30 days in any criteria and/or exhausting your CDA is grounds for dismissal from ESA and your family would be dis-enrolled from ESA. Cumulative files would be forwarded..
